Introduction

The intent of this survey is to assess your current knowledge, skills and attitudes with respect to Fall Prevention Initiatives in the community pharmacy setting prior to attending the educational seminar. Your participation is voluntary and your refusal to participate will involve no penalty, and you may discontinue the survey at anytime at no risk to you.  Your responses will be kept confidential. If you elect to proceed, you will be asked a series of questions. This survey should take approximately 15 minutes to complete.
