RSVP form for Community Vision Tables

This is a registration form to attend one of four planned Community Vision Tables. We estimate this could take between 4-9 minutes to fill out.

The Community Vision Tables are open to members of the public who live, work, go to school or recreate in the greater Portland region - including Washington, Clackamas, and Multnomah counties.

All attendees can receive an $80 stipend as an acknowledgement of their time.
