Informed Consent Process
The New Zealand Health and Disability Commissioner Act came into law in 1994
WavesNZ has received hundreds of anecdotal complaints from families
who claim their rights under the HDC Act have NOT been upheld.

Survey Objective:
1) Collate complaints on behalf of families
2) Identify the HDC rights which are consistently disregarded
3) Engage with the HDC and insist the Commissioner follow due process
so that Informed Consent is upheld

For the purposes of this survey, an “event” is a time when vaccination,
(Referred to in the MOH Immunisation Handbook as a medical procedure)
was discussed and/or administered.

If there was more than one event,
please record each one in a separate survey form.

Collated results will be made available on the WavesNZ website.
