Thank you for participating in the Family Child Care Provider Administrative and Business Needs Survey!

Your response will help shape the first cooperatively owned network of family child care providers. The goal of this cooperative is to make your work, and the work of other family childcare providers, easier and better. Your answers to this survey will help design services and supports to meet your most urgent needs, whether that means help maintaining enrollment, managing paperwork, hiring substitute staff or carving out space for personal time.

 

This survey should take 10 to 15 minutes to complete. It will be used to understand the most common needs of family child care providers in Illinois. Your name and other personally identifying information you provide in this survey will not be shared or made public. We will only contact you with your permission. As a thank you for your time and effort, by completing this survey you will be entered into a drawing for a $200 Visa gift card. 

The Family Child Care Provider Administrative and Business Needs Survey was developed by The ICA Group, a national non-profit whose mission is to expand economic opportunity and self-determination by supporting initiatives that empower workers, build community assets, and root capital locally.
