Building / Zoning Violation Complaint Form

Please fill out the requested information as completely as possible. Only submit a complaint against one property at a time. If you wish to file complaints against multiple properties, you must fill out a separate complaint form for each property. Complaints on one form against multiple properties will be disregarded.

Anonymous or pseudonymous reports are not accepted. Confirmation of reporting party via phone or email must be completed prior to investigation. If enforcement staff are unable to validate a reporting party's contact information, the complaint will be disregarded.

A reporting party's name and contact information will be kept strictly confidential. However, should this complaint become part of a criminal and/or civil court case, this information may be provided to the District Attorney or any such other appropriate authority as part of their investigation.
