Thank you for taking the time to help us learn more about the community's needs related to opioid use and how we can better address those needs. This survey, funded by The Center at Sierra Health Foundation's Communities of Color grant, is aimed at gauging what the community already knows about opioids, as well as the community's knowledge of treatment options and the stigma surrounding treatment. We appreciate your honesty when completing the survey to ensure our resulting education development and actions are truly beneficial. Please feel free to visit our website before you take the survey to learn more about opioids, Substance Use Disorder and locally available treatments. This will help us determine what educational opportunities we are still missing.

Upon completion of the survey, participants will have the opportunity to submit their email address to be entered into a raffle to win 1 of 10 prizes to local restaurant and retailers. Please do not take the survey more than once, your answers will not be counted again.
