Disability Inclusion Action Plan Review Survey

We are developing our next Disability Inclusion Action Plan (DIAP) and we want your input.

This survey helps Council understand what is working well, what barriers exist, and what improvements are most important for people with disability, carers, and the wider community.

It should take about 5-10 minutes to complete. You may choose to skip some questions; however, a number are required so that we can gather important information to better understand community needs and identify opportunities for improvement. All responses are anonymous and confidential.
