Focus Group - Tuesday, November 10, 7pm-8:15pm

Your answers to the following questions are to help determine whether you qualify to serve on a focus group. Some of the questions are personal in nature, but all information you provide about yourself during this survey and during the focus group is kept strictly CONFIDENTIAL.

Please answer all questions fully. You may decline to answer a question, but declining to respond may result in your not being selected for this focus group.

If you are selected, someone will contact you WITHIN 3-4 DAYS to confirm your availability, and you will be paid $60 for a 1.25-hour session when the session is over in exchange for your participation.

This survey will take approximately 6-7 minutes to complete. You must AGREE to the questions below, or you will be disqualified from participating in the focus group.
