COVID-19: Waiver and Release of Liability
The Friends for Life Bike Rally (FFLBR) /Toronto People With AIDS Foundation (PWA) are committed to complying with the applicable requirements and recommendations of National, Provincial and local public health and other governmental authorities in respect of COVID-19. Nevertheless, FFLBR/PWA cannot guarantee that you will not become infected with COVID-19. Further, attending the 2022 Friends for Life Bike Rally and any related activities, programs, or services (the “Rally”) could increase the risk of contracting COVID-19, despite preventative measures.
The Friends for Life Bike Rally (FFLBR) /Toronto People With AIDS Foundation (PWA) are committed to complying with the applicable requirements and recommendations of National, Provincial and local public health and other governmental authorities in respect of COVID-19. Nevertheless, FFLBR/PWA cannot guarantee that you will not become infected with COVID-19. Further, attending the 2022 Friends for Life Bike Rally and any related activities, programs, or services (the “Rally”) could increase the risk of contracting COVID-19, despite preventative measures.
By signing this document,
1) I declare that I am participating in the Rally voluntarily.
2) I declare that neither I nor anyone in my household has been diagnosed with COVID-19 and not yet been cleared as non-contagious by public health officials. If I or anyone in my household is diagnosed with COVID-19 after submitting this declaration, I agree that I will not attend the Rally (including any related activities, programs, or services).
3) I declare that neither I nor anyone in my household has experienced cold or flu-like symptoms in the last 10 days (including fever, cough, sore throat, respiratory illness, or difficulty breathing). If I or if anyone in my household experiences any cold or flu-like symptoms after submitting this declaration, I agree that I will not attend the Rally (including any related activities, programs or services) until at least 10 days have passed since those symptoms were last experienced.
4) I declare that I will adhere to the requirements and recommendations of National, Provincial and local Public health and other governmental authorities and to any special safety regulations implemented by FFLBR/PWA in respect of COVID-19.
5) I acknowledge and understand the highly contagious nature of COVID-19 and that being exposed to or infected by COVID-19 could lead to illness, injury, or death. I acknowledge and understand that the risk of being exposed to or infected by COVID-19 by participating in the Rally may result from the actions, omissions, or negligence of myself and others, including, but not limited to, FFLBR/PWA, their employees, agents, and volunteers, Rally participants, and their families. I voluntarily assume the risk that I could be exposed to or infected by COVID-19 by participating in the Rally, and accept sole responsibility for any resulting injury, illness, damage, loss, or expense of any kind.
6) In consideration of the acceptance of my participation in the Rally, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby waive, release, discharge, and agree to hold harmless FFLBR/PWA, their directors, agents, volunteers, employees, sponsors, and representatives, and any other person or organization assisting in the Rally from any and all actions, causes of action, claims, commitments, promises and demands whatsoever which I now have or hereafter may have that in any way arises in connection with my exposure to or infection by COVID-
