Introduction

Thank you for contributing information to the Hospital-Initiated Recovery Programs Dashboard. The Virginia Department of Behavioral Health and Developmental Services (DBHDS) State Opioid Response (SOR) grant team and OMNI Institute maintain this dashboard to educate legislators, program directors, and the larger community about Virginia’s hospital-initiated recovery programs, including peer recovery support programs based in the emergency department (ED) and other hospital departments, and Medication Assisted Treatment (MAT) Bridge programs, which offer MAT services in emergency departments for patients with opioid use disorder. We appreciate any information you can provide!

  • If you are submitting information on a new hospital, not currently included in the dashboard, please answer all information that you have about the program(s).
  • If you are submitting edits to a hospital currently on the dashboard, feel free to complete only the fields that need updating.
  • If you are unsure of an answer, please leave that question blank. Surveys that are partially completed will be included in the dashboard.
  • If you would like to share information about multiple hospitals, please complete a separate survey for each hospital. If you have information about many hospitals, feel free to contact SORRecovery@omni.org so that we can help streamline data entry.
  • By entering information in this survey, you agree that it can be shared publicly through the dashboard. If you would like information removed from the dashboard, please contact SORRecovery@omni.org.

If you have any questions about the survey or additional information you’d like to share, please reach out to us at sorrecovery@omni.org. Thank you for your time and contributions!
