Welcome to the Tribal COVID-to-Climate Questionnaire

You have been invited to participate in the Tribal COVID-to-Climate Questionnaire being led by the Pala Band of Mission Indians' Tribal Climate Health Project, with funding from the National Indian Health Board's Climate Ready Tribes program. Completing the questionnaire will take approximately 30 minutes. Thank you for participating.
Why is this analysis being done?
The purpose of this questionnaire is to collect tribal experiences, knowledge, best practices, and lessons learned while responding to and recovering from the COVID-19 pandemic that may be translatable to strengthening tribal preparation for emergencies resulting from climate change.  The findings will be used to inform a report that will be distributed to Tribes and tribal-serving organizations nationally.
Who can I contact if I have questions about this analysis?
If you have any questions or concerns about the research, please contact Prosper Sustainably's Angie Hacker (ahacker@prospersustainably.com).
What are my rights as a participant?
Your participation in this analysis is entirely voluntary. Whatever decision you make, there will be no penalty or loss of benefits to which you were otherwise entitled. You may refuse to answer any questions that you do not want to answer. You may also choose to withdraw from the analysis at any point during your participation (i.e. not complete the questionnaire) without consequence. You may refrain from sharing any information or traditional knowledge you deem privileged or confidential.
