Welcome to the application for the Union for Reform Judaism Communities of Practice (CoP). We ask that you complete and submit this application by May 23rd, 2016. Here are some helpful hints:

1. You may exit the survey and return to it by completing the page you are on, clicking next so that the information is saved, and then using the same computer to click on the link and log back in.

2. If your congregation is applying to more than one CoP, you will need to submit separate applications- most questions are unique to each CoP being offered. Also, the link can only be used once from any particular computer, so we encourage each CoP "team leader" fill out their respective applications.

3. If you want to look at the application questions in advance or work on composing your answers offline, you can log in to The Tent, go to the Communities of Practice group, and find a printable version of these questions.
