Page 1 of 2: Kansas City All-access Card Youth Survey

The Center for Community Health and Development at the University of Kansas is conducting a feasibility assessment of an all-access pass to activities and services in the city, similar to the My Denver Card in Denver, CO, and the DC One Card in Washington D.C. Youth and adults could use the card to access existing services, programming, and activities, and receive referrals relevant to their goals and interests. Please complete this survey to assist in determining the level of interest in and potential benefits of an all-access card system.

If you have any questions about this service please contact Jerry Schultz, Ph.D., at KU’s Center for Community Health and Development - jschultz@ku.edu or 785-864-0750.
