Purpose of this survey

Retina International wish to understand and report on the impact of COVID-19 to the retina patient community. Based on findings from our initial COVID-19 survey, this survey aims to identify specific accessibility issues exacerbated by COVID-19 and the associated restrictions.

This survey will collect personal information about you including demographic information (such as your age and gender at birth and country of residence). It will also collect your personal opinion about access to everyday activities including your use of healthcare services.

Your information will be used to describe characteristics of groups of people (not individuals) who face the challenges associated with access to services or resources for those impacted by a visual impairment.

Confidentiality

Your personal information collected as part of this survey will be kept strictly confidential. Your identity will not be revealed and your confidentiality will be protected in any reviews and reports of this study which may be published.

While we use a third party survey platform provider to assist us in administering this survey, this provider is bound by strict confidentiality and privacy obligations. This means that your information will only be accessed by authorised personnel where required for the purpose of administering the survey and conducting the analysis described above.

Voluntary participation/right to refuse or withdraw

Your participation in this survey is voluntary; and you can withdraw at any time if you change your mind. If you decide to withdraw your consent by exiting the survey mid-way through, or by contacting orla.galvin@retina-international.org, or the person who conducted the survey with you, your responses will not be recorded.
