Initial Assessment

Completing this pre-assessment is an important step in the process through which people who are at-risk of becoming homeless can gain access to the best options to address their needs.

* 1. Have you obtained a notarized letter from the person who you are staying with requesting you to move out by a certain date? Your notarized letter must be attached to this pre assessment to be considered eligible for services. The notarized letter must be written by your friend/family member and must include the following information: the date, applicant's name (which is you), friend/family address and phone number, must state it is a temporary situation, and you must leave.
