Youth Attitudes to Vaccines

The Immunize Colorado Youth Advocates are sending out this survey to understand their peers attitudes towards COVID vaccines, routine vaccines, and what barriers have been formed to prevent their peers from being vaccinated. Please respond to the best of your ability and honestly.
