Introduction

The purpose of the Equity Action Plan is to better understand what your organization has identified as priorities that you would like to work on over the next 24 months to advance equity within your organization and within the overall system of care in King County to improve health outcomes and address health disparities.

You will be asked to identify one goal within each of the following categories - 

1.   Advancing equity for the individuals that you serve or those who work with, and for, your organization;
2.  Advancing equity by embedding equity within your organization as a way of doing business; and,
3.  Advancing equity in the way that the overall system of care operates.

Each section has a description and examples of possible goals.  While examples are included, they are offered only to prompt idea generation.  The Equity Action Plan is designed to honor where your organization is along its equity journey.  We recognize that some organizations are farther along than others.  Thus, the Plan is individualized and we encourage you to develop goals and strategies that are:

1.   Specific and focused to achieve milestones;  
2.  Measurable through the use of both qualitative and quantitative information;
3.  Attainable - selecting something that is within your organization's control; 
4.  Relevant - something that will help YOUR organization advance along your equity journey; and
5.  Time-based - something that you realistically achieve within the next 24 months.
