2026 Administrator of the Year Award Nomination Form

This award category is available to nursing facility members only. This award honors an administrator from a member nursing home who is nominated by his or her employees, peers, owner or facility official. The purpose of the award is to recognize an administrator for his or her outstanding efforts. The nominee must be the administrator of the facility.
