2026 Middle Waves Vendor Application

Middle Waves Music Festival is a regional music festival held at Headwaters Park in Fort Wayne, IN. Along with live music, the festival features a vendor area, game zone, art zone, and a selection of food trucks.

Completing this form does NOT guarantee participation. If selected, vendors will receive an invoice and will be considered officially registered once payment is received. Booth spaces will be held for up to 30 days after the invoice is sent, through May 1. After May 1, booth spaces will only be reserved with immediate payment. Due to limited space, we encourage vendors to complete payment promptly to secure their spot.

No MLM or direct sales company vendors will be allowed.

Vendor Details and Guidelines:
  • Vendor may NOT sell beverages
  • The registration fee for vendors is $100.
  • Spaces are approximately 10x10.
  • You are to provide your own table and chairs.
  • You can bring a tent. Weights must be used.
  • Registration is non-refundable unless the festival is canceled by Mother Nature.
  • You must be set up no later than 2pm. (Time subject to change.)
  • No electricity provided
  • Please attach W-9 when you send to your payment. We can provide a blank one if you need us too.
The deadline to register is Friday, May 1st.

Additional information will be sent to you about loading in/out parking prior to the festival.
