Thank you for taking the time to complete this survey. The City of Seattle's Department of Education and Early Learning (DEEL) is gathering input and feedback from young people like you across Seattle about youth mentoring programs.
Your feedback will help shape program to help young people stay connected to school and employment. Please know that questions in this survey may ask about sensitive topics such as experiences with justice systems, coping with stress, and feeling safe at school. This survey is voluntary and you can opt out and stop at any point. This survey should take approximately 10-15 minutes to complete.
You might be wondering about the attendance questions - don’t worry, you won’t get in trouble for sharing if you’ve missed school. This survey and focus group are only meant to help us understand what makes it hard for youth to attend regularly. DEEL isn’t part of Seattle Public Schools, and your responses won’t be shared with principals or district staff. What you share will only be used to improve programs, not for discipline or enforcement.

At the end of the survey, you can enter a drawing for a $50 e-gift card . 20 e-gift cards are available.

Because this survey is collected by a city agency, it may be considered a public record under Washington State law (RCG Chapter 42.56). This means that if someone were to formally request survey responses, we could be required to share them but any information that could identify you, like your name or contact details, would be removed first.
