Title of Study: Individual Preferences and Characteristics

Investigators: Renee Murray, Zlatan Krizan, Kevin Blankenship

This is a research study. Please take your time in deciding if you would like to participate. If you have any questions please feel free to e-mail Renee Murray at rmurray@iastate.edu. This study has been approved by the Iowa State University Institutional Review Board (#10-465).

The purpose of this study is to examine relationships between personal characteristics and preferences. You are being invited to participate in this study because you are responding to an ad/request to complete the study that you viewed online.

If you agree to participate in this study, your participation will last less than 50 minutes. During the study, you will be asked to complete several measures assessing your personal characteristics and preferences. Please keep in mind that you may skip any question that you do not wish to answer or that makes you feel uncomfortable.

There are no foreseeable risks beyond those normally encountered in daily life. However, if you feel uncomfortable at any point you may immediately stop without penalty.

If you decide to participate in this study there may be no direct benefit to you. However, it is hoped that the information gained in this study will benefit society by providing a greater understanding about the relationships between individual characteristics and personal preferences.

You will not have any costs from participating in this study. As a way to thank you for your participation, you will be given the opportunity to enter into a drawing for a $25 amazon.com gift certificate at the end of the study. Your chances to win the drawing will depend on the number of participants who complete the study, but we estimate the chance of winning to be approximately 1 in 300.

Your participation in this study is completely voluntary and you may refuse to participate or leave the study at any time. If you decide to not participate in the study or leave the study early, it will not result in any penalty or loss of benefits to which you are otherwise entitled.

Records identifying participants will be kept confidential to the extent permitted by applicable laws and regulations and will not be made publicly available. However, federal government regulatory agencies, auditing departments of Iowa State University, and the Institutional Review Board (a committee that reviews and approves human subject research studies) may inspect and/or copy your records for quality assurance and data analysis. These records may contain private information.

To ensure confidentiality to the extent permitted by law, the following measures will be taken: (a) you will be assigned a unique code and letter and will be used on forms instead of your name, (b) your data will be combined with data collected from other participants so that no individual information can be identifiable, and (c) all records will be either kept in a locked filing cabinet or password protected computer files. Only the investigators listed above will have access to study records. Data may be kept up to seven years. If the results are published, your identity will remain confidential.

You are encouraged to contact the investigator at any time during or after the study if you have questions.
