- School district mailing address
- Ohio county for the school - do not put "Ohio"
- The State Board of Education District Number Go to the Department OTOY webpage and scroll down to find the SBOE District Lookup Tool. *If you are a community school please contact angela.dicke@education.ohio.gov to help confirm your SBOE District information.
