About this Award
The Leukemia & Lymphoma Society (LLS) recognizes a professional leader for their career of support for patients with blood cancer and ongoing involvement with The Leukemia & Lymphoma Society. The Hematology-Oncology Lifetime Acheivement Award is presented at the opening session of the Association of Oncology Social Work (AOSW) National Conference.
Submissions must contain
  • A completed nomination form
  • Nominee's resume or curriculum vitae
  • Letters of support (minimum of two, no more than four) addressing criteria for the appropriate award from other social work professionals, co-workers, hospital administrators, or community agency representatives. The letters of support should address the nominee's demonstrated professionalism, integrity and commitment to oncology social work.
