BACKGROUND INFORMATION

You are being invited to participate in a research study directed by Kimberly LeBlanc (PhD Nursing student at Queen’s University, thesis work under the supervision of Dr. Woo) to determine the impact of living with an ostomy in Canada. The Queen’s University Health Sciences and Affiliated Teaching Hospitals Research Ethics Board have reviewed this study for ethical compliance. I am contacting you through the Canadian Ostomy Society and via Ostomy Supply Manufacturers. 

THE STUDY

The purpose of this study is to explore how Canadians are impacted financially by having an ostomy and are there differences by province. The Canadian Association for Enterostomal Therapy (CAET) plans to use the results of this survey to lobby federal and provincial governments for improved funding and access to specialized care for individuals living with an ostomy. You will be considered for this study if you are 18 years of age and over and living with a temporary or permanent ostomy in Canada and consent to participate. You will be asked to complete a short and confidential survey.

RISKS

There are no foreseen risks associated with this study. All responses will remain confidential and all materials will be stored in a secure and password protected server. Data will be destroyed 5 years after the termination of the study as per the University of Queen’s policy.

BENEFITS

While you may not benefit directly from this study, results from this study may improve the understanding of living with an ostomy in Canada and the Canadian Association for Enterostomal Therapy (CAET) plans to use the results of this survey to lobby federal and provincial governments for improved funding and access to specialized care for individuals living with an ostomy.

EXCLUSIONS

You will not be considered for this study if you: are under 18 years of age or do not currently have an ostomy.

CONFIDENTIALITY

All information obtained during the course of this study is strictly confidential and your anonymity will be protected at all times. There will be no personal identifiers on any data. Data will be stored in locked files and will be available only to principal investigator. You will not be identified in any publication or reports.

VOLUNTARY NATURE OF STUDY/FREEDOM TO WITHDRAW OR PARTICIPATE

Your participation in this study is voluntary. You may withdraw from this study at any time and your withdrawal will not affect your future medical care with your physician or at this facility.

Consent will be implied by participant’s completion and submission of the completed survey via Fluid Survey’s online site. At no time will the researcher have access to the names, contact information or email addresses of the participants. All personal identifiers will be removed from the data prior to download by the researcher from the Fluid Survey database.
