Survey for individuals with family members in long-term care during COVID |
While quarantine measures are a necessity to protect residents of long-term care facilities, we are seeking to understand the impacts these measures are having on the residents, outside of protection from COVID-19, so that we may formulate ways to address any problems secondary to quarantine measures during similar future quarantine scenarios.
This survey is voluntary and anonymous. You will not be asked for your name or anyone else's name. You are not required to participate and may stop at any time. Completing this survey should take about ten minutes. The information from this survey will be used to determine if there are any secondary negative effects of COVID-19 quarantine measures on residents or long-term care workers and to identify possible solutions to these problems in future quarantine scenarios.
After completing this survey you will be asked if you would like to follow up and complete an anonymous telephone interview with researchers to offer more information on how COVID-19 quarantine measures in long-term care facilities are affecting residents and workers in those facilities. This too is optional and voluntary.
