About this Survey
Your voice matters! We want to hear what it was like for you to look for or use mental health and addictions services for infant, children and youth ages 0–18 in Lambton County. What made it easy or hard to get support? Did you get the help you needed? What would you change?
Your ideas can help us make services easier to find, easier to use, and better suited to what young people and their parents or caregivers need. Your answers will help shape our 2027-30 Service Area Plan—our plan for improving local infant, child and youth mental health and addictions services.
The survey takes about 15 minutes.
Taking part is your choice
You decide whether to take this survey. You can skip any question or stop at any time. Taking part—or choosing not to—will not affect the mental health and addictions services you receive now or in the future.
Your privacy matters
The survey is anonymous. You do not need to share your name or any information that identifies you. Responses will be stored in SurveyMonkey, where access is limited to people authorized to view them. We will share a summary of the results, without identifying anyone. The results will be combined with other information and shared with community partners to help plan infant, child and youth mental health and addictions services and shape the 2027-30 Service Area Plan.
Thanks for sharing your experience!
At the end of the survey, you can fill out a ballot for a chance to win a $50 Lambton Mall gift card.
