Your Child, Your Family, Your Voice Matters!
Parent/Caregiver Survey about Infant, Child & Youth Mental Health and Addictions Services

About this Survey

Your voice matters! Help us make it easier, help us make it better, help us design a service system that makes sense to youth and parent/caregivers! As someone who has recent experience reaching out for and/or using infant, child and youth mental health and addictions services (0-18 years old) in Lambton County, we want to learn more about what the experience was like for you.
  • Did anything get in the way of reaching out for services or using services?
  • Did you get the help you needed?
  • What ideas do YOU have to improve child and youth mental health services?
Your answers to this survey will help to inform our 2027-30 Service Area Plan to ensure that services are easy to find and use, as well as available and responsive to the mental health and addictions challenges children and youth are experiencing in our community.

This survey will take about 15 minutes to complete.
