This Final Report should be completed one time per organization after all events in your contract are completed and will be reviewed by CalMHSA for approval.

Tabling at the LACDMH event in your contracted Service Area should also be completed before the Final Report is completed.

Please do not submit your final invoice until you have received approval of your final report.

Before completing this Final Report, please review your grant application, contract, implementation and marketing plan, and expenses, in addition to the data collected from your event(s).(Required.)
