Dear Parent/Family Member of Summer Rising Student:
New York City Public Schools invites all Summer Rising parents, caregivers, or family members to complete this online survey. The survey will help New York City Public Schools (NYCPS) understand how the summer program is helping your student and what could improve the summer program.
Your survey answers will be anonymous and only seen by the researchers hired by NYCPS to help with the survey. None of your responses will be seen by CBO and NYCPS site staff. Your names, emails, and phone numbers will not be collected or connected to your responses. Your responses will not affect your ability to access Summer Rising programming this summer or in the future. The researchers will combine the survey information before sharing it with NYCPS.
Participation in this survey is voluntary; you may skip any question you do not want to answer. The survey will take approximately 10-15 minutes. Please note that you must be 18 or older to take the survey. If you have questions about the survey, please contact Donna Wilkens at dwilkens@metisassoc.com. For questions about your rights as a survey-taker for this research study, please contact the NYCPS IRB at IRB@schools.nyc.gov.
The survey will close at 5 p.m. on August 14th.
