Dear PARENTS/GUARDIANS,
The Hawai'i State Department of Education (DOE) invites you to participate in the School Quality Survey (SQS) for the 2025-2026 school year. Your feedback about your child's school is invaluable and will help the DOE and school leadership assess and enhance the quality of education in our schools. All responses will be kept strictly confidential. Please submit the completed survey by March 12, 2026.
Survey Instructions:
* The survey will take approximately 8-10 minutes to complete.
* Please complete the survey in one sitting, as responses cannot be saved and resumed later.
* You may respond to one survey for each child. You will begin the survey by inputting your child's first, middle, and last name initials and your child's grade level. (If your child does not have a middle name, input "#".)
* Most questions require agreement responses ranging from "Strongly Agree" to "Strongly Disagree." Select "Neutral" if you neither agree nor disagree, and "N/A" if you are unsure or the statement does not apply to you.
Language translations (15 languages) are available for the SQS parent letter from the Superintendent Keith Hayashi and for the questions on this survey. You may access the translations at arch.k12.hi.us/reports/sqs (Please see the parent letter for directions.)
