We are asking people who live in Dover, Medfield, Needham, and Sherborn to help us identify key health needs in your community.

The data we collect will help Dover, Medfield, Needham, and Sherborn understand the region’s strengths and challenges related to health services, helping to ensure resources are directed where they are needed most. You can find answers to Frequently Asked Questions here. The survey is part of a Community Health Assessment (CHA) by the Charles River Public Health District. The questions in the survey were informed by findings in the 2026-2030 CRPHD Strategic Plan.
  • The survey will take about 15 minutes to complete.
  • The survey is intended for residents of Dover, Medfield, Needham, and Sherborn who are age 18 or older.
  • Your participation in the survey is voluntary.
  • You are welcome to skip any questions you do not feel comfortable answering.
  • All responses to the survey will remain confidential. Data from the survey will be combined for aggregate analysis and reporting.
  • The survey will remain open until June 1, 2026.
