Health and Well Being of 2026 Masters Field Hockey World Cup Athletes
Informed Consent
This briefly explains the study and asks for your agreement to participate.

NATURE AND PURPOSE: I am being asked to participate voluntarily in the above-titled research project. The purpose of this project is to increase our understanding of health and wellbeing of masters field hockey athletes participating in the 2026 Masters Hockey World Cup (outdoor).
PROCEDURES: If I agree to participate, I will be asked to complete a confidential survey that asks about me, my health, my health behaviors, my participation in field hockey, and my wellbeing.
RISKS: There are no risks to participating. I understand that I am free to stop participating in this project at any time I wish.
BENEFITS: There is no direct benefit to me. The study will assist us with examining the health and wellbeing status of master’s field hockey athletes.
CONFIDENTIALITY: All responses will be kept confidential. All reports will be prepared in a way that protects the anonymity of participants.
PARTICIPANT COSTS & SUBJECT COMPENSATION: The only cost to me will be my time. Completing the survey will take about 5-7 minutes. I will not be compensated for my participation.
