Important Notes Before Starting
We strongly encourage you to use the Survey Workbook or other document to collect responses and award nominations offline, then enter everything into the online survey all at once. SurveyMonkey uses a cookie stored in the web browser on the device you use to start the survey. That cookie is what remembers your answers — if you switch devices or web browsers (or clear cookies), your responses may not be retained. Whenever possible, complete the survey in a single session. If you must pause, stay on the same device/browser and do not clear cookies.
What Companies are Eligible: Companies with operations, at least 5 employees and community engagement programs in the designated local survey region are eligible to participate.
What Data is Kept Private: Points of Light and OneOC publicly recognizes the top 50 honoree companies and award recipients, but does not disclose any company-specific data. Companies that participate but do not qualify as honorees remain confidential.
What Questions are Scored: Not all required questions are scored. Questions marked in the Survey Workbook with an orange star (★) will be scored.
