2025 DCCF Hero Awards - Nomination Form

Please nominate a Douglas County Hero for consideration for the Douglas County Community Foundation 2025 Hero Awards.

The Armed Forces Hero Award recognizes outstanding individuals or organizations in the Douglas County Armed Forces and veteran community.

The Community Hero Award recognizes an outstanding nonprofit organization, community leader, volunteer, or corporation located in Douglas County.

The First Responder Hero Award recognizes outstanding individuals or organizations in Douglas County law enforcement, including police officers and sheriff deputies, firefighters, emergency medical services personnel, dispatchers, search and rescue, and volunteer first responders.

Nominees must have responded to a situation exhibiting exceptional courage, extraordinary compassion, or lifesaving actions considered above and beyond the call of duty. They can have administered extraordinary emergency aid, or performed outstanding volunteer community service of a sustained, direct and consequential nature.

The events must have occurred in the last 18 months.

Online nominations will be accepted through Thursday, August 7, 2025, at 12:00 p.m. Final nominees will be invited to attend the DCCF Heroes Gala on September 12, 2025, and will be honored during the event.

This category includes active-duty military, veterans, and organizations of the sort. Nominees should have demonstrated exceptional courage, self-sacrifice, extraordinary compassion, or life-saving actions that go above and beyond the call of duty.

10.For Community Hero Award:
This award honors an exceptional nonprofit organization, community leader, volunteer, or corporation based in Douglas County. Nominees should have made a significant impact on charitable initiatives through financial contributions, the deployment of organizational resources, encouraging employees to give their time and resources to the Douglas County community, or a combination of the above. Outstanding volunteer community service that is sustained, direct, and impactful is also eligible for consideration.

This category includes civilian and commissioned law enforcement personnel (police officers, sheriff deputies), firefighters, emergency medical services personnel, dispatchers, search and rescue, and volunteer first responders. Nominees must have responded to a situation showcasing exceptional courage, selflessness, extraordinary compassion, or life-saving actions that go above and beyond the call of duty, including providing remarkable emergency assistance.
