1. ATTEST: Please attest to your participation to claim AOA Category 1-A Credits or AMA PRA Category 1 Credits. This form is mandatory to complete if you wish to have your credit reported to the AOA or AMA on your behalf.
2. REPORTING TO AOA or AMA: Your information will be reported to the AOA or AMA on your behalf by the completion of this form.
3. FILL OUT AND PRINT YOUR CERTIFICATE: Once you have completed the CME statement of attendance, you will be directed to a page where you can fill out and print your own CME certificate. Please note your total number of hours to fill out your certificate accurately.
