Guided Pathways: Submit Updates to a 2025-26 Program Map Form

Instructions:

To request an update to a program map, the Department Chair (or Designee) will need to complete and submit the form below.

For reference, access the Area of Study (AoS) Program Maps listing via ACC Catalog Program Maps - please be sure to choose the 2025-26 College Credit Catalog.

NOTE: This form is only to update a current program map. To view information about creating a new program map, please first visit the Program Maps Process page and then the Guidelines for New Program Maps page for additional information.

Additionally, department contact information needs to match the program pages accessed via the ACC’s Academic & Career Programs website --- please submit a Web Service Request form if any updates are needed to your program page in the college’s Academic & Career Programs website.

Please Note: Any changes to the department name, award plan title, award type, course information/credit hours, and prerequisite/corequisite courses must first be approved by the Curriculum & Programs Committee before the program map can be updated.
