2024 Skate Ontario AGM Proxy Form

Eligibility:

In accordance with By-Law 3.16- Proxy Voting- Every Member entitled to vote at a meeting of Members may, by means of a proxy, appoint a proxy holder to attend and vote on behalf of the Member. Each proxy holder shall be a delegate and may carry a maximum of six (6) proxies.

A separate form must be complete for each proxy.

Timelines:

Skate Ontario requests that all Delegate forms be completed by Sunday, September 15, 2024 at 11:59pm ET/10:59 CT.

By Thursday, September 19, 2024, confirmed proxy holders will receive login instructions for Skate Ontario's webinar and voting platforms. Proxy holders will be required to login Saturday, September 21, 2024 by 9:15am ET/8:15am CT. The meeting will commence at 9:30am ET/8:30am CT on Saturday, September 21, 2024.
