2024 Zion Baptist Church Graduate Recognition Form

Instructions

We will recognize Zion members and non-members who are graduating or being promoted from any level between preschool through post-graduate, as well as from other programs (e.g., Ushering School) during our Graduates Day celebration following the worship service on Sunday, June 2, 2024. To be included, by Sunday, May 19, 2024, you must:

(1) Submit all the information on the form below; and

(2) Send a photograph of the graduate to the College Ministry either via email to CollegeMinistry@ziondc.org or via U.S. mail to Zion Baptist Church College Ministry, c/o Deacon Minette Barnes, 4850 Blagden Avenue, NW, Washington, DC 20011. Please include the Graduate's name on the back of the photo.

Please note that if you are a Zion member who is a graduating high school senior AND submitting a Zion Baptist Church Scholarship Application, you do not need to complete this form. Instead complete the application form at https://www.surveymonkey.com/r/2024ZBC-ScholarshipApp.
