The Way Home Partnership Application 2024

Hello! Thank you for your interest in becoming a member organization of The Way Home. The Way Home is the name of our local Continuum of Care (CoC); which is the collaborative effort underway to make homelessness rare, brief, and non-recurring in the cities of Houston, Pasadena, and Conroe, and throughout Harris, Fort Bend, and Montgomery counties in Texas.

The Coalition for the Homeless of Houston/Harris County (Coalition) serves as the Lead Agency and Homeless Management Information System (HMIS) administrator for The Way Home. The Coalition has developed this membership application/partner update to gather accurate and updated information about those engaged in ending homelessness throughout our region.

Please note that this is not an application for those experiencing homelessness. If you are currently experiencing homelessness or at immediate risk of homelessness, please contact the Coordinated Access line at ca@homelesshouston.org.

Each agency/organization should select a primary contact who is in a leadership position to complete this application. The information provided may be made available as a resource within our CoC (e.g., on The Way Home website as part of a list of resources for people experiencing homelessness seeking assistance).
