About the Award

The Award recognizes individuals who have demonstrated exceptional leadership and made outstanding contributions in the areas of education, practice, administration or policy in post-acute and long-term care. This leadership is characterized by results of increased visibility of critical issues, creation of solutions to significant problems, and positive impact on the overall quality of care in post-acute and long-term care.

Criteria:
  • Exceptional leadership and outstanding contribution in Education.
  • Exceptional leadership and outstanding contribution in Clinical Practice.
  • Exceptional leadership and outstanding contribution in Impacting Policy (State or Federal) of Post-Acute and Long-Term Care Services.
  • Exceptional leadership and outstanding contribution in Administration
  • Exceptional leadership and outstanding contribution in Creating Meaningful Solutions to Problems in Post-Acute and Long Term Care.
  • Exceptional leadership and outstanding contribution to the Quality of Care in Post-Acute and Long-Term Care settings.

SUBMISSION REQUIREMENTS:

1)  Complete submission form;
2)  Upload all three (3) required supporting documents:
  • One (1) brief nomination letter; an introduction to the nominee, including the nominee's merits, and the rationale for nomination;
  • Two (2) additional letters, from separate individuals, supporting your nomination.
Award nominations will be reviewed by the CALTCM Award Selection Committee, Executive Committee, and the Board of Directors.

Nomination deadline is March 20, 2024.
