Dear LHD Commissioners, Directors, and Deputies:

NYSACHO is pleased to offer each of you a NYSACHO swag item of your choosing. We want to thank you all for your continued hard work each and every day to keep our communities safe. We will be distributing these gifts at the 2023 Public Health Leadership Summit in Watkins Glen, NY. Don't worry! If you can't make it to the conference, we will mail your gift to you. 

Branding is an important component of NYSACHO's Strategic Plan. By showing off your new gear in your home community, you’ll help raise awareness of NYSACHO’s profile locally.

We have six NYSACHO swag items for you to pick from (a polo, three-quarter sleeve, yeti tumbler, fleece pullover, fleece zip-up, or insulated tote)! We also have two bonus items you can select (a baseball cap or beanie). Please complete the following survey to select which item you want.
