2021-22 State Seal of Biliteracy Participation Form

The purpose of this form is to:

I. Indicate the district's participation in the 2021-22 State Seal of Biliteracy program;

II. Identify the world languages in which students will demonstrate proficiency and their associated assessments; and

III. Verify information in NJSMART to confirm students’ eligibility for the State Seal of Biliteracy.

Please note that this form should only be completed by the district (e.g., Biliteracy Coordinator) and that once the form is started, it cannot be saved prior to submission.
