The 2022 FAST Global Summit & Gala Ticket Giveaway is open to parents OR grandparents of individuals with Angelman syndrome who are members of FAST and intend to attend both the 2022 FAST Global Summit and FAST Gala, Mission Possible.  If you are not a registered member of FAST, click here and then return to the survey.   Each family may enter to win either one (1) or two (2) tickets  to the 2022 FAST Global Summit & Gala taking place on December 2nd & 3rd, 2022 in Hollywood, Florida.  Please submit only one entry per family - duplicate entries will result in both entries being deleted.
 
Please enter correct and accurate contact and address information.  If you are selected as a winner, we will contact you through the email address you provide on this form. A registration link for ticket winners will be emailed to you.
 
Please use proper address format (capitalize the first letter of first name, last name, guest name, street, city, state and country).  If you live outside of the United States, please include country code when entering your phone number.
 
There is a very good chance you will win tickets in this drawing, so please only enter if you plan to attend the 2022 FAST Global Summit & Gala.
