Intro: Your Homecare Worker Feedback

I’m in a power chair after a fall in 2012 broke my neck, and I became a "quad." I utilize a great team of about 6 homecare workers for about 85 hours per week, here in our home with my wonderful wife Debra in southwest Eugene.

This is a brief, private, free survey for homecare workers in Eugene.

First, this survey helps me to keep track of homecare workers interested in joining our team, either as either with a regular worker shift or as a sub.

Second, this survey helps build the community of homecare workers in Eugene. To thank you for completing this survey, I will email you a free resource page about Eugene homecare workers. 

Even though all of the info here will be kept private, if you indicate in the survey, I can share data with other disabled people in Eugene looking for homecare workers.
