The Many Faces of Community Health Conference Planning Committee is in the process of preparing for yet another annual conference. This will be the 16th year of the Many Faces of Community Health Conference, which will take place virtually on November 4-5, 2021 via PheedLoop and Zoom Webinar.

The Many Faces of Community Health Awards are intended to recognize individuals and organizations that have been at the forefront of leading innovations in health care, improving care delivery models, and promoting health equity. The effects of COVID-19 and social injustices continue to negatively impact communities across the state. Given this year’s theme, Leading Change in a New Normal, we would like to pay special recognition to those who have been involved in transformative community health leadership efforts this past year.

We are pleased to announce that the nomination period for these three annual awards is now open! We are inviting you to submit nominations for one or all of the award categories. Select an award on the following page to learn more about eligibility and selection criteria, and to submit a nomination. Nominations are due by COB on Friday, September 3rd for the three awards:

  • Outstanding Community Health Center Employee
  • Visionary Leader in the "New Normal"
  • Telehealth Innovator

To be considered, nominations must be received by close of business on Friday, September 3rd, 2021.

Award winners will be recognized during the 2021 Virtual Many Faces of Community Health Conference. Winners will receive a complimentary registration to attend the conference.
