Purpose of Value Nominations
Our Augusta Health Values enrich our culture of care by enhancing how we each deliver our service and expertise to our patients, their families, our community and, of course, each other.
Quarterly, we hold a celebration of an individual or small group who went above & beyond in their demonstration of one value in particular, out of our four core values of Patient & Community Centeredness, Excellence, Professionalism, and Teamwork. This is where you come in!
To successfully complete and submit this short nomination form, you'll be asked to:
1) IDENTIFY the individual or group (up to 6 people),
2) SELECT one value they best represented, and
3) SHARE detailed examples of how they went way beyond expectations to impact those we serve. In order to be considered for this competitive award, specific actions taken by each nominee must be included.

NOTE: Inspired by your nominee's demonstration of more than one value or was a value role modeled by more than one individual or group, you can submit as many unique nomination forms as desired!
