To express interest in one of the Advanced Life Support (ALS) - 2 day courses held at Fiona Stanley Hospital in 2020, please complete the below registration form.
 
You will then be placed into an applicant pool for the date(s) you selected, and will be notified 6 - 8 weeks prior to the course if your application was successful/unsuccessful with the selection panel.
 
Medical Staff: Professional Development Leave applications will only be accepted once the Medical Education Unit confirms you have been selected to attend the course.
Nursing and Midwifery: Please ensure you discuss your leave requirements with your line manager.
 
IMPORTANT: completion of this form is NOT confirmation of acceptance onto a course.
