2021 Awards - District Governor's Citation and Club Awards Submission
General Award Submission Rules:
Utilize this survey to enter each award category submission individually (once you finish the submission, close and re-open survey to submit additional categories)
Use the Master Checklist to indicate what is being submitted and submit individually.
For each category, please complete the two page submission- checklist and one page narrative- and submit.
Please do no alter the form; any additional information will not be considered.
You can include up to six (6) pictures to support your entry for each category with your submission of the narratives. Please make sure all pictures are on one page per narrative. Simply upload the six photos along with the checklist/narrative.
