Introduction

Welcome to the 2021 CFMA Chapter Benchmarking submission form. This form will be used to calculate points for Chapter Benchmarking and Chapter Awards.

Please keep the following in mind when completing your submission -
  • Once you begin submitting your information you must complete the form.  You cannot complete a portion of the form and finish at a later time. To submit this information faster, it is strongly recommended that you complete the Excel worksheet version (found on the Chapter Benchmarking and Awards web page) and use it to fill in this online form.
  • All questions on this form are being asked of the April 1, 2020 - March 31, 2021 year.
  • For each question, the point value is shown. Please enter the information asked for and not the point value. The point value is included for reference only and should not be the answer to a question.
All submissions are subject to review by the Chapter Resource Committee and point totals may be adjusted. If an adjustment is made or if there is a question regarding your submission, you will be contacted.
