2021 CALTCM Leadership Award

Please complete this form and submit a brief letter of introduction listing the nominee's merits and the rationale for nomination, along with two letters of support. Leadership Award submissions will be reviewed by the CALTCM Award Selection Committee, Executive Committee, and the Board of Directors.

The CALTCM Leadership Award recognizes individuals who have demonstrated exceptional leadership and made outstanding contributions in the areas of education, practice, administration or policy in post-acute and long-term care. This leadership is characterized by results of increased visibility of critical issues, creation of solutions to significant problems, and positive impact on the overall quality of care in post-acute and long-term care.

Criteria:
  • Exceptional leadership and outstanding contribution in Education.
  • ŸExceptional leadership and outstanding contribution in Clinical Practice.
  • ŸExceptional leadership and outstanding contribution in Impacting Policy (State or Federal) of Post-Acute and Long-Term Care Services.
  • ŸExceptional leadership and outstanding contribution in Administration
  • ŸExceptional leadership and outstanding contribution in Creating Meaningful Solutions to Problems in Post-Acute and Long Term Care.
  • ŸExceptional leadership and outstanding contribution to the Quality of Care in Post-Acute and Long-Term Care settings.
SUBMISSION REQUIREMENTS:

1)  Complete submission form;
2)  Upload all three (3) required supporting documents:
  • One (1) brief nomination letter; an introduction to the nominee, including the nominee's merits and the rationale for nomination;
  • Two (2) separate letters supporting your nomination.
Leadership Award nominations will be reviewed by the CALTCM Award Selection Committee, Executive Committee, and the Board of Directors.

Nomination deadline is March 10, 2021.
